annual affiliation form
The Annual Affiliation Form must be complete before every seasonal year by each organization.
Complete the form in your organization’s GotSport Account.
1. Login at system.gotsport.com
2. Select 'Affiliations' from top menu bar
3. Select 'Available Forms (2)' and the Eastern Pennsylvania Youth Soccer Annual Affiliation Form for the appropriate year
4. Hit the 'Select' button to view and complete the form for the seasonal year
