2023-24 Certificates of Insurance (COIs) Request Form Below:
Complete the form below to provide owners or operators of game and practice facilities proof of liability insurance before allowing member organizations to use those facilities. Standard COI requests take 2-3 business days to process. If special wording is needed, the processing time will be more. Please ensure you allow ample time for your request.
Organizations making requests must be Eastern Pennsylvania Youth Soccer members in good standing with an Annual Membership Affiliation Form on file. All COI requests must be made by the organization's Insurance Coordinator or President.
As of September 1, 2022, bulk completed certificates will be sent to the Club President. Additionally requested COIs will be sent to the Insurance Coordinator for the organization. As a reminder, this individual is required to be listed in the organization's GotSport Club Admin portal. It is their responsibility to make sure the property owner receives the certificate.
If you are requesting a certificate for a commercial facility, Eastern Pennsylvania Youth Soccer will only issue a certificate if the commercial facility is a sanctioned facility.
